Service Agreement and Consent Forms
Our agreement and consent forms are tailored specifically for virtual counseling and advocacy services. These documents are essential to ensuring that you receive the support you need safely and confidentially.

DARCC offers comprehensive services for those whose lives have been affected by sexual violence.
Services are available to all survivors 13 years and older, as well as to individuals impacted by sexual violence through their relationship with a survivor.

Virtual simply means that services are administered through internet-based technology. Appointments will be held through your smartphone or computer using Zoom, a HIPAA-compliant service.
